Whether the mother nurses sitting up or lying down, she holds the baby flat against her body, facing her, so that he does not have to reach or twist his neck to latch on. Whether the areola is large or small, the milk sinuses lie about the same distance behind the nipple, a distance matching the size of a newborn baby’s mouth. While the baby is on the breast, his tongue and jaws apply pressure to the sinuses, not to the nipple itself. The nipple’s well back in the baby’s mouth, where it is virtually untouched during nursing.
She hardly thinks of it; she may not even be aware of it. Her baby wants to nurse, she gives him the breast, and the milk comes. It is infallible. Only when the baby begins to lose interest in the breast, which will not occur for at least nine or ten months (and usually not until after the first birthday), does her milk supply diminish and her letdown reflex operate later in the feeding and finally fade away. If the baby were to catch a cold or feel the need for extra comfort for a few days at this point and want the breast more often, both the milk secretion and the letdown reflex would return.
Hospital milk banks, which collect and store human milk for premature and sick babies, have found that almost any woman who cares to be a donor can produce surplus milk beyond her own baby’s needs. Some milk bank donors produce a few ounces of surplus milk a day, others donate a pint or more. Such individual variations seem to be entirely with the mammalian safety margin that ensures that every baby has more than enough milk. Some women can produce at least twice as much milk as is needed by the average baby; most can nurse twins.
